Home Health Care

Skilled Nursing

Our Registered Nurses are assigned as case managers to ensure you receive a complete assessment and holistic care.  We are trained to provide you with a wide range of nursing care procedures in your home. Here are some of the modules that Skilled Nurses are able to provide you with:


  • Catheter Care and change
  • Congestive Heart Failure Management
  • Diabetic Care and Teaching
  • Ostomy Care
  • IV infusion Therapy and Injections
  • Wound Care 
  • Medication Management and reconciliation
  • Orthopedics such as knee and hip replacement
  • COPD & Pulmonary rehab
  • Neurological care in Dementia, Parkinson's, and Stroke


Prior to providing you with any of the above listed services, the nurse will conduct a skilled admission assessment. He or she will evaluate your health condition, recommend a treatment regimen with guidance from your physician and help develop a nursing care plan that can be administered in your home. Physical Therapy

Physical Therapy involves thorough evaluation of physical functioning and sound recommendation for a treatment that focuses on rehabilitation. This is what our physical therapists at Home Care Select Home Health will do for you. Even more conveniently, we provide physical therapy in your home.  Our goal is to prevent falls, improve physical function, and allow you or your loved one to safely age in place as long as possible.

  • Effective Energy Management and Conservation

Sometimes, a patient’s condition may be too severe for rehabilitation. In such cases, Home Care Select will provide you with an occupational therapist who can administer treatment that focuses on helping you manage your disability. We can help patients cope with their mobility challenges through medical equipment, mobility-assistive devices and other methods specific to the condition at hand. 

Medical Social Worker

Someone in your family has recently been diagnosed with diabetes, heart failure or COPD.  Your grandfather may be struggling with memory impairments. Your parent may be facing disability after an injury. In these situations, you will take the role as caregiver – but you may not be able to do it alone. The tasks can be overwhelming, however, you can get emotional and administrative support from professionals at Home Care Select.


At Home Care Select, we have Medical Social Workers who can assist you during health crises in your family. 

Our MSW can enhance the quality of your care at home by addressing the psychosocial needs of patients and their families related to illness, disability, treatment and care. We work as a team with other clinical personnel and community providers in providing comprehensive health care planning on behalf of the patient, helping you and your family to make the best use of available medical, emotional, interpersonal and community resources.


We are compassionate listeners and creative problem solvers. We can serve as mediators and we help identify possible solutions and alternatives. 

Speech Therapy

It can be difficult to live normally if you have a speech or hearing impediment. Some clients have communication difficulties brought about by the aftermath of a stroke, an injury or may be congenital in nature.

Regardless of the reason, there are remedies that you can turn to. At Home Care Select Home Health , we recommend communication-challenged individuals for Speech Therapy. The treatment normally includes but is not limited to:
